Ninety percent of adult Singaporeans experience some level of gum disease, which can vary from mild cases like gingivitis to more severe forms such as periodontitis. This article will discuss the top causes of gingivitis, its key symptoms, and possible treatment methods.

What is Gingivitis?

Gingivitis is a common and mild form of gum disease characterised by inflammation of the gingiva, the part of your gum surrounding the base of your teeth. This condition often results in redness, swelling, and bleeding of the gums, particularly during brushing or flossing.

The primary cause of gingivitis is the accumulation of plaque, a sticky film of bacteria that forms on the teeth. When plaque is not removed through regular brushing and flossing, it hardens into tartar, which can irritate the gums and cause gingivitis.

Treating gingivitis is crucial because it can prevent the condition from progressing to periodontitis, a more severe form of gum disease that affects the deeper structures of the gums and supporting bone.

If left untreated, bacteria continues to accumulate, causing the gums to become more inflamed. This inflammation can extend deeper into the supporting tissues, leading to bone loss around the teeth. As the bone deteriorates, the stability of the teeth is compromised, potentially resulting in tooth loss.

Gingivitis is the initial, milder form of gum disease, characterised by inflammation of the gums. At this stage, the damage is primarily superficial, and with proper oral hygiene and treatment, the condition is reversible.

In contrast, periodontitis is a more advanced stage of gum disease that develops if gingivitis is left untreated. It involves the destruction of the supporting structures of the teeth, including the bone. Periodontitis may be irreversible and requires more intensive treatment to manage the condition and preserve oral health.

Causes of Gingivitis

Gingivitis is mainly caused by plaque accumulation, which hardens into tartar, irritating the gums. The development of gingivitis can be exacerbated by several risk factors:

  • Poor oral hygiene: Inconsistent brushing and flossing allow plaque to accumulate.
  • Smoking or tobacco use: This can impair gum health and reduce blood flow.
  • Hormonal changes: Events like puberty or menopause can increase gum sensitivity.
  • Weak immune system: Conditions like AIDS and leukaemia can cause gingival bleeding.
  • Medications: Certain medications like antidepressants and oral contraceptives can increase susceptibility to gingivitis
  • Genetic susceptibility: Some people have a predisposition to gum disease.

How Gingivitis is Diagnosed

When evaluating a patient for gingivitis, the dentist will first take a detailed medical history, which includes questions about oral hygiene practices, symptoms, medications, and any underlying health conditions. Following this, a physical examination of the mouth is conducted, where the dentist will check for signs of swelling, irritation, and plaque buildup on the gums and teeth.

Next, the dentist will measure the pocket depth of the gums using a periodontal probe. This measurement helps determine the extent of gum detachment from the teeth. In cases where severe swelling or detachment is observed, the dentist may also recommend dental X-rays for closer examination of the underlying bone structure.

Professional Gingivitis Treatments

This section covers treatments going beyond standard home care, specifically those administered by a dental professional.

Scaling and polishing is a non-surgical treatment option designed for mild cases of gum disease, such as gingivitis. This procedure focuses on removing plaque and tartar buildup from the teeth and gums.

The process consists of two main steps:

  1. Scaling: In this first step, a dentist uses specialised instruments to meticulously remove plaque and tartar from the tooth surfaces, both above and below the gum line.
  2. Polishing: After scaling, the dentist will polish the teeth using a soft brush and a mild abrasive paste. This step smooths the tooth surfaces, making it harder for plaque to accumulate and enhancing the overall appearance of the teeth.

Root planing is a treatment option designed for more severe cases of gingivitis, particularly when pockets of infection have formed between the gums and teeth.

The process involves deep cleaning below the gum line, where harmful bacteria often reside. Plaque and tartar is removed from the roots of the teeth, smoothing their surfaces and removing remaining bacteria. By thoroughly cleaning and reshaping the roots, root planing also reduces inflammation and allows the gums to reattach to the teeth.

Another possible minimally invasive option would be laser periodontal gum treatment. Specifically, we use Fotona Twin Light Therapy, where lasers of two different wavelengths (Nd:YAG and Er:YAG) create optimal conditions for healing periodontal tissues.

This treatment involves 3 simple steps:

  1. The Nd: YAG laser removes the diseased epithelial lining of the periodontal pocket, improving access to the root surface.
  2. The Er: YAG laser is used to thoroughly eliminate microbial biofilm and calculus from the root surface.
  3. The Nd: YAG laser energy is applied to coagulate tissue and promote the formation of a stable fibrin clot.

Laser treatment eliminates the need for incisions, leading to less bleeding, swelling, and discomfort.

Gingival flap surgery is an invasive procedure used to treat severe cases of gingivitis that have progressed to periodontitis. This surgical intervention is necessary when non-surgical treatments, such as scaling and root planing, are insufficient to manage the disease.

During gingival flap surgery, the dentist lifts the gums away from the teeth to access the underlying bone and root surfaces. A thorough cleaning of the infected areas and the removal of any damaged tissue is then conducted. Once the area is cleaned and treated, the gums are repositioned and sutured back into place.

Bone grafts are used in dentistry to replace lost bone in the jaw or to treat gum recession. Both applications are invasive procedures.

Grafting typically becomes necessary when bone or tissue loss occurs due to advanced gingivitis. This procedure involves surgically placing the graft in the affected area, allowing the bone or tissue to integrate with the existing structure.

Effective Home Gingivitis Treatments

This section covers simple remedies for treating gingivitis using items that are readily available around the house or at the drugstore.

Saltwater has natural antibacterial properties, making it an effective home remedy for soothing inflamed gums and promoting oral health.

To create a saltwater rinse, simply mix half a teaspoon of salt in a cup of warm water. Stir the solution until the salt is completely dissolved. Swish the saltwater around in your mouth for about 30 seconds before spitting it out. This rinse can be used several times a day and helps reduce inflammation and combat gingivitis.

Oil pulling therapy involves swishing a tablespoon of oil (coconut, sesame, or sunflower oil) in the mouth for about 15-20 minutes. As the oil is swished, it binds to the bacteria and plaque, pulling them away from the teeth and gums. By reducing the total oral bacterial count, gingivitis can resolve faster.

Mouthwashes containing chlorhexidine are powerful antimicrobial agents. Chlorhexidine not only reduces the overall bacterial count but also helps prevent the formation of plaque, making it particularly useful in managing gingivitis.

Medications such as antibiotics can significantly reduce bacteria levels in the mouth by targeting and inhibiting their growth. In the context of gingivitis, antibiotics can be particularly effective in managing severe cases where conventional treatments may not suffice.

However, antibiotics are typically reserved for severe cases of gingivitis due to the potential risk of antibiotic resistance.

  • Gum and tooth discomfort
  • Red, swollen gums
  • Bleeding gums during brushing or flossing
  • Persistent bad breath
  • Receding gums
  • Loose teeth

However, if gingivitis persists or worsens despite good home care, it’s crucial to seek professional dental treatment. Persistent symptoms may indicate that more advanced intervention is needed to address the underlying issues.

Fluoride toothpaste is considered the most effective option for managing gingivitis. Fluoride not only helps strengthen tooth enamel but also helps kill the bacteria that contribute to plaque formation. By reducing harmful bacteria in the mouth, fluoride toothpaste minimises plaque buildup, which is a key factor in the development of gingivitis.

You can tell if your gingivitis is healing by observing several positive changes in your gums. One of the first signs is that your gums will feel less swollen and tender to the touch. As inflammation decreases, they should feel firmer instead of soft and puffy.

Additionally, you may notice a significant reduction in bleeding during brushing and flossing. If you previously experienced blood while cleaning your teeth, less or no bleeding indicates that your gums are becoming healthier.

Gingivitis can turn into periodontitis relatively quickly if left untreated. It takes just about four days for plaque to reach its peak accumulation on the teeth. If gingivitis remains unaddressed, it can progress to periodontitis in as little as 2 to 3 weeks.

Gingivitis itself is not contagious, meaning you cannot directly catch the condition from someone else. However, the bacteria that cause gingivitis can be transmitted through saliva during mouth-to-mouth contact, such as kissing or sharing utensils. This means that while you cannot catch gingivitis directly, you can be exposed to the bacteria that contribute to its development.

Therefore, it’s important to prioritise oral hygiene not just for individual health, but also to reduce the risk of spreading harmful bacteria to others.
